I'm struggling to find it, but I remember watching a video relatively recently of a mechanic who was working on a blown motor. Turns out the owner didn't remove the plastic wrap from their oil filter before they installed it. The threads cut the plastic, the oil carried some of it into the oil passages, and it blocked a critical passage to lubricate the camshaft.
They found it when disassembling the motor to figure out everything that was broken.
Sometimes stupid can be profitable. Most of the time it's just extremely frustrating to deal with. This one was both.
