Hi, I'm Doug with an '06 VR3.6L Passat 2WD.
The top mechanic in town (also a Ross-Tech customer, of course) replaced the throttle pedal; the VW Dealer where I bought the car new replaced the TB (Throttle Body) electrical connector; and the top VW dealer mechanic in our region has no diagnosis. So now, my town mechanic gave me the fault codes and suggested I try this forum:
000289 - Throttle Position Sensor (G69): Implausible Signal
005476 - Throttle Actuator (J338): Under-Voltage during Basic Setting
001287 - Idle Control System RPM: Higher than Expected
The title above summarizes the problem: idle hunt, accompanied by occasional misfire, which is followed by a surge in RPM. The surge often trips the check engine light for open loop mode, and occasionally the surge trips into limp home (20MPH max) mode.
Temperature is a major factor. Cold start or air temps below 50 Fahrenheit seem to have less problems; warm start or air temps above 90 Fahrenheit can result in immediate failure to limp home mode. Octane is a secondary factor: 87 octane seems to be more problematic, 93 octane seems to be less problematic.
I read on the Ross-Tech Garage forum that O2 sensors are wired into the TB circuit on the 2.0L motor, and could cause these kinds of problems without throwing an O2 fault code. I've also read that a low battery voltage could be the cause.
Any suggestions on a cost effective approach to replacing parts, lowest cost with highest likelihood of success??? I can turn a wrench; heck I even completely rebuilt a V8, but that was a 1970 Chevy with a 4-barrel carb and points.

Ok, mechanic re-ran Autoscan, after failure into openloop mode, and even more fault codes were found. Here's the current scan:
UPDATE 2:
Immediately after running the above autoscan, here's what happened. With outside air temp 33F and motor temperature gauge at 160F, motor started with ESP and Antiskid Lights, but I believe it was in closed loop mode. Engine missed everytime I attempted to accelerate, could surge up to 2000 RPM, then miss and fall to below 500 RPM before firing again. After about 60 seconds, the motor temp gauge increased to 200F and the car performed perfectly; the only Light on was Check Engine, but otherwise perfect acceleration and its 800 RPM idle was absolutely steady and smooth.
My drive home was for two miles, and the car has now been parked for about 10 minutes. I'm about to take another short 4 mile trip, and if any further symptoms I can post them.
UPDATE 3:
After the 10 minutes of being parked, I started the engine and it idled smooth at 750RPM. No lights, even the Check Engine light was off. However, with my foot on the break pedal and shifting into Drive, the load on the engine resulted in a mis-fire and the engine died. This is a new symptom, and these symptoms which started over a year ago are very gradually becoming more severe. After restarting the engine, I held the accelerator pedal at 1000 RPM before shifting into Drive, and the car performed perfectly again, excellent acceleration, no Check Engine light, and smooth idle.
